Travel Consultations
We now offer travel consultations. These are generally not covered by insurance. We charge $100 payable at the time of appointment. This fee does not include any additional immunizations or medications that your provider may feel are neccessary for travel.

Typhoid Vaccine
We now have Typhoid injectable vaccine. The cost is $95, paid at the time of service. Children must be 2 years old to receive this vaccine and the current guidelines suggest that it will last 2 years.

This questionnaire must be completed before your Physical Exam (age >6years) or before your physician can sign any activity/camp/sports forms.
