Whether you are expecting a newborn, have recently moved to our area, or are looking for a new medical home for your children, we thank you for trusting us with your child’s health!
We welcome new patients starting at birth to age 18 years. Established patients can be seen up to age 21 years.
Newborns
We look forward to meeting your infant at their first newborn visit! We like this appointment to be in the first three days of life. Please call us after your baby is born to schedule an appointment. If you are a new family to us, please read through the bios of our pediatricians to choose a doctor who you think might be the best fit.
New Patient Resources
- In preparation for your first visit we have bundled all forms to be completed in the New Patient Registration Packet.
These forms can simply be printed as they exist and filled out by hand. However, they would then need to be faxed, or mailed to us.
These forms can be filled out electronically provided you either have the free Acrobat Readersoftware, or have a web browser with the appropriate PDF plug-in enabled.
Once the form is filled out, you should save it on your computer in a location you will remember. Then log on to the NSP Patient Portal (My Kids Chart) and send a message with the saved form(s) attached.
