Convenient Access to Health Records
We will communicate with you via the portal for lab results and non-urgent communication in order to link all communication within your child’s chart.
From the patient portal, you can review a complete health information summary for each child in your care, including:
- Most recent well child visit date
- Upcoming appointments
- Historical visits
- A summary of labs and medical tests
- A problem list, allergy list and medications list
- A complete immunization record, available to download
- Growth parameters, available to download
- All visit summaries are provided online. We do not print our summaries unless requested.
Send a Secure Message
Secure messaging is a feature of our patient portal. You can use it to contact us about non-urgent medical questions, medication refills, and appointment requests.
13 Years and Older Release Form
When a patient turns 13 years old in the state of Washington, by law, the record for that patient automatically becomes private.
After the patient is 13 years old, he or she may grant permission to a parent or guardian to have access to the chart by completing and signing this release form. This permission can be revoked at any time at the request of the patient or at the discretion of the physician.
